Why the CAT Gen 7 Is the Only Tourniquet in Our Kits

I have been a paramedic for 23 years. I have applied a lot of tourniquets. I have also seen a lot of bad ones fail at the worst possible moment. So when it came time to choose what goes in Module 1 - Hemorrhage Control, I did not just grab whatever was popular or cheap. I went back to the research, compared it against what I have seen in the field, and made the call based on both.

Here is what I found.

The CAT Gen 7 is proven 100% effective at occluding arterial blood flow in both upper and lower extremities. That comes straight from the U.S. Army Institute of Surgical Research. Not a marketing claim. Documented military research. The U.S. Army has been using it as their official tourniquet since 2005 and here is what happened after that. Battlefield deaths from extremity hemorrhage dropped from 23.3 per year to 3.5 per year. That is an 85% reduction. And research across 238 combat casualties showed that when you apply a tourniquet before a patient goes into hemorrhagic shock, survival rate hits 90%. Without it, that number drops to 10%.

In 2019 JAMA Surgery published research by Goolsby and colleagues. They took 102 people with zero medical training, people who had never touched a tourniquet in their lives, and had them apply five different tourniquets. I know this sounds obvious and these were not paramedics or soldiers. These were regular people. Exactly like the person who might open your kit in an emergency.

CAT Gen 7: 92.2% success
SOFTT-W: 68.6% success
Improvised tourniquet using a belt or cravat: 32.4% success
RATS bungee device: 11.8% success
SWAT-T elastic wrap: 11.8% success

The RATS and the SWAT-T each hit 11.8% with untrained users. Out of 100 people trying to save someone's life with one of those devices, only about 11 of them would actually get it right. That is a very scary number. And the person bleeding out in front of them is counting on that working. That is not a small gap. That is a life-or-death gap.

Imagine trying to stop a fire hose with a rubber band. That rubber band just does not have the strength to squeeze it closed completely. That is what an elastic tourniquet is doing when you apply it to a large limb arterial bleed. It compresses the soft tissue but it cannot generate enough pressure to fully occlude the artery. The bleeding slows but it does not stop. And in arterial hemorrhage, slowing is not enough.

A 2025 systematic review in Advances in Simulation confirmed this across every metric they measured. Elastic tourniquets performed worst in occlusion, worst in blood-loss control, worst across the board. None of them are on the CoTCCC Recommended Devices list. Not because nobody has heard of them. Because they did not meet the performance standard required to get on that list.

Now the counterfeit problem.

Fake CAT-style tourniquets from China are all over Amazon, eBay, and bulk kit sellers. They look almost identical to a genuine Gen 7. The difference shows up when you actually need it to work.

In controlled testing, counterfeit CATs generated less than 25% of the occlusion force of a genuine Gen 7. The windlass is cheap plastic. It bends or snaps under the pressure required to stop arterial flow. The buckle fails. In 2013 two counterfeit tourniquets failed at the scene of a motorcycle accident in New Hampshire. First responders documented it. The U.S. International Trade Commission issued a General Exclusion Order blocking importation of fake CATs. North American Rescue has taken legal action against counterfeit manufacturers.

A genuine CAT Gen 7 from an authorized dealer costs between $25 and $30. It is made in the United States. If you paid significantly less than that from a third-party seller or an Amazon multi-pack, I would go ahead and replace it. Your life is worth more than the difference in price.

Here is how to spot the real one. Pick yours up right now and check these five things.

1. Windlass rod. Deep raised ribbing with raised CAT lettering at both ends. Try to flex it with your hands. A genuine Gen 7 does not budge. A counterfeit bends or feels hollow. That rod is what you are cranking down on someone's limb in an emergency. If it snaps you have nothing.

2. Single-feed routing buckle. Raised CAT lettering molded directly into the buckle. Run your finger across it. You should feel it not just see it. Smooth or painted-on lettering is a flag.

3. Windlass strap. Sonic welded to the clip, not stitched. Look for the weld mark where the strap meets the clip. If you see stitching there it is not genuine.

4. Red elliptical tip. At the end of the free-moving internal band. Sonic welded. Not glued. Not stitched. Welded.

5. Lot information. Manufacture date and NSN number printed on the tail end of the hook and loop band near the red tip. Missing or smudged lot information is a major flag.

Everything I have said above applies to humans. But I want to address the SWAT-T specifically because on Thursday I am going to tell you it belongs in the K9 kit, and I do not want that to seem like a contradiction. It is not a contradiction. It is anatomy. Think of a dog's leg like an ice cream cone. Wide at the top, narrow at the paw. Now try to put a rigid clamp on an ice cream cone and tighten it. It slides right off toward the tip. That is exactly what happens when you apply a CAT Gen 7 to a dog's leg. The windlass cannot seat correctly against that tapered shape. K9-TCCC guidelines state it explicitly. Human windlass tourniquets including the CAT Gen 7 are generally ineffective in K9s due to conformational differences and should not be used as first line therapy.

The SWAT-T elastic design conforms to that conical shape. It wraps, it molds, it holds where a rigid windlass cannot. Same tourniquet that fails on a human limb is exactly what works on a dog's limb.

So the CAT Gen 7 is for humans. The SWAT-T is for animals. Both are correct. Neither is interchangeable. I will go deeper on the K9 side on Thursday.
